Eden House Long-Term Care Home is a 58-bed facility licensed by the Ministry of Long-Term Care, providing 24-hour resident-centred nursing and personal care. We are not a large institutional facility — we are an intimate, home-like setting where staff know every resident by name.

Situated in a beautiful countryside setting between Rockwood, Eden Mills and Guelph, Eden House provides a peaceful, dignified environment where residents can live as independently as possible, with the full support they need.

24-hour resident-centred nursing and personal care provided by Registered Nurses and Registered Practical Nurses. On-site physician supervision and regular medical rounds.

On-site pharmacy services and daily medication assistance ensure residents receive their prescribed medications safely, correctly and on schedule every day.

Nutrition & Dining

Three nutritious meals per day with a menu-style service, plus morning, afternoon and evening snacks. Registered dietitian assessment, therapeutic diets and supplement programs.

Personal Care

Assistance with all activities of daily living — bathing, dressing, grooming, toileting and mobility — provided with dignity, patience and genuine compassion by our PSW team.

Housekeeping & Laundry

Daily housekeeping keeps resident rooms and common areas clean and comfortable. Personal laundry and bed linens are laundered daily and returned to residents.

Recreation & Activities

Full-time recreation team delivering daily programming — daytime and evening. Music, arts & crafts, exercise, social events, community outings and seasonal celebrations.

On-site dental services are available for residents, ensuring oral health care is accessible and convenient without requiring transport to outside appointments.

How to Apply

Admissions Process

Placement in a long-term care home in Ontario is coordinated through your local Home and Community Care Support Services (formerly LHIN). Here's what to expect.

Contact Home and Community Care

Begin by contacting your local Home and Community Care Support Services (HCCSS) to request an assessment for long-term care placement. They will evaluate eligibility and care needs.

Complete the Assessment

A care coordinator from HCCSS will conduct a thorough assessment to determine your level of care needs and eligibility for long-term care. This assessment determines the care you require.

Choose Eden House

Once approved, you can choose up to five long-term care homes. We encourage you to visit Eden House in person and meet our team before making your selection.

Wait for a Bed to Become Available

Your placement coordinator will contact you when a bed becomes available at Eden House. You have 24–48 hours to accept the offer of a bed.

Move In & Settle In

Our team will welcome you and your family on move-in day and help you settle into your new home. We conduct a detailed care conference within the first few weeks to review your individual care plan.
